FORMS YOU WILL NEED TO COMPLETE

Our law firm will require that you provide several completed forms as we begin your representation. First, you must sign a retainer agreement, detailing our fee schedule. Second, you should fill out an interview sheet that includes data such as the length of your marriage, your children's name and ages, your employment history and your spouse's employment history. Finally, the court requires that you provide a financial affidavit, a comprehensive statement of your monthly expenses and monthly income.

You may find these forms below. Please complete these forms prior to our initial conference. You may print these forms, complete them and fax back to us at 770-642-7578 for our review prior to our meeting or you may simply bring them with you to the initial conference if you prefer.
